Optus, Australia's second largest telecoms carrier, experienced a 13-hour disruption of emergency call services on an unspecified date in November 2023. This failure, linked to four deaths, occurred during a routine network firewall upgrade and affected approximately 600 customers. The company attributed the disruption to a departure from regular processes during the network upgrade. Following the incident, Optus confirmed that at least five customers attempted to report the outage, but call centre operators did not escalate these concerns. The incident led to the resignation of CEO Kelly Bayer Rosmarin. Stephen Rue subsequently assumed the role of chief executive and pledged to improve services.
